New Jersey Covered Person Opt-Out (Daniel’s Law)
Pursuant to New Jersey law (P.L. 2023, c.113, et al.), this form is for “Covered Persons” or their authorized representatives to request the non-disclosure of their home address and unpublished home telephone number.
A “Covered Person” includes:

• Active, formerly active, or retired judicial officers, law enforcement officers, child protective investigators, or prosecutors.

• Immediate family members residing in the same household as one of the above.

Submitting this form serves as a formal request to cease disclosure and remove the protected information. Supporting documentation is not required at this time.
